Indications and Results of Thyroidectomies in Northern Senegal  [PDF]

Abstract: Goiters account for a very high prevalence disease. In Senegal, surgery is a very important part of the management. Our aim is to determine indications and results of thyroidectomies at the regional hospital of St. Louis. Patient and Method: We conducted a descriptive retrospective study over a period of 6 years on 81 records. All patients who underwent thyroidectomy during the study period were included. Computer software EPI INFO and Excel were used for collection and processing of data. Results: The average age of our patients was 40 years with a sex ratio at 0.05. The determination of TSH found 88.9% in euthyroid, 1.2% in hypothyroidism and 9.9% in hyperthyroidism. Thyroid ultrasound was performed in 97.5% of patients. Surgically, gestures performed were divided between the lobo-isthmectomies (43.1%), subtotal thyroidectomy (9.8%), total thyroidectomy (46.91%). The postoperative course was uneventful in all patients. The histology was benign in 88.9% of surgical specimens and found adenocarcinoma in 11.1%. Conclusion: Our study confirms the predominance of female thyroid pathology. The thyroidectomy which is getting better codified occupies an important place in the management. It offers the advantage around the problem of therapeutic non-compliance and also stockouts in our region.

Abstract: Introduction: COVID-19 is a global public health emergency that can cause acute respiratory distress syndrome. In countries where tuberculosis (TB) is endemic, coinfection of COVID-19 and TB is often encountered, which increases the risk of developing severe forms of COVID-19. Objectives: The aim of this study was to determine the prevalence of COVID-19/TB coinfection at the Epidemic treatment center (ETC) in Saint-Louis (Senegal) and to describe the epidemiological, clinical, paraclinical, and outcome profile of co-infected patients. Patients and Methods: This is a retrospective, cross-sectional, descriptive cohort study based on the records of COVID-19/ TB co-infected patients who were hospitalized at the ETC in Saint-Louis (Senegal) over an 18-month period from March 2020 to September 2021. Results: Out of a total of 454 hospitalizations, we collected records of 9 patients co-infected with COVID-19/TB, resulting in a prevalence of 2%. The study included patients with a median age of 34 years (range: 10-86 years), with a male predominance (7 cases) and a sex ratio of 3.5. The majority of patients (88.9%) had severe forms of COVID-19. Dyspnea and cough were reported in all patients (100%). Pulmonary TB was the most frequent localization, with 9 cases. The diagnosis of COVID-19 was confirmed by nasopharyngeal PCR in all patients (100%). Bacilloscopy was positive in 3 out of 5 cases. One patient tested positive for GeneXpert? MTB/RIF without rifampicin resistance. All patients were prescribed the hydroxychloroquine-azithromycin combination and anti-tuberculosis treatment. Out of the nine patients, four recovered (44.4%) and five died (55.5%). Conclusion: COVID-19/TB coinfection had a low prevalence in our cohort, but was associated with a high mortality due to the frequent occurrence of severe forms of the disease.
Analysis of Rainfall Dynamics in Conakry, Republic of Guinea  [PDF]

Abstract: Observed rainfall data of the National Meteorological Service of Guinea (NMS) exhibit that synoptic station usually records the largest rainfall amount in Guinea. Only few studies have been done on this rainfall peak observed in Conakry. This work better analyses the atmospheric dynamics leading to rainfall particularity. Using NMS data from 1981 to 2010, the monthly contribution and mean seasonal cycle of each station has been done. These findings of the study show that between July and August (rainfall season peak), the coastline particularly Conakry records the largest amount of rainfall. Using Era Interim data for the common period (1981-2010), we also investigate the rainfall dynamics in the lower level (1000 hPa - 850 hPa) from precipitable water, divergence, and moisture flow transport. There is a west and southwest moisture flow transport explained by a strong moisture convergence in the coastal region (Lower-Guinea). Furthermore, values of precipitable water in the same region are found, in agreement with the high moisture flow transport gradient. These incoming flow (west and south-west) undergo a return by blocking’s Kakoulima range (foehn effect) and Fouta Djallon massif to initiate convection clouds on the Guinean coast. These processes enhance a convergence of moisture associated with orographic origin convection. This has an important effect by increasing the rainfall amount in Conakry.

Abstract:
The most common vascular complication of acute pancreatitis is thrombosis of the splenic vein. Isolated thrombosis of the superior mesenteric vein is rare and may lead to mesenteric ischemia and bowel infarction. We report the case of a 39 years old patient received for acute pancreatitis with a Ranson score less than 3 and image scanner for a grade C of Balthazar and a superior mesenteric vein thrombosis. The outcome was favorable with improvement of symptoms under medical treatment including anti-coagulant therapy. The contrast enhancing abdominal CT showed an absence of superior mesenteric thrombosis.
Evaluation of Water Resources Quality in Sabodala Gold Mining Region and Its Surrounding Area (Senegal)  [PDF]

Abstract: Geochemical and Geostatistical tools were used to assess: 1) the chemical quality and, geochemical processes in crystalline rock aquifers in Sabodala (Eastern Senegal) and its surroundings and 2) the impact of mining activities on their quality. A total of 26 water samples collected at boreholes, dug wells and stream, were analyzed to determine major and trace elements concentration focused on elements that represent more threats on human health. Boxplots define chemical characteristics of water for each aquifer formation compared to surface waters. Geostatistical analysis show two sources of water mineralization with regard to major elements: a first natural source characterized by Ca-Mg-HCO3 water type from boreholes and unpolluted surface water and a second group characterized by polluted water by nitrates with Na-NO3-Cl type mainly observed in upper weathered aquifers. However, considering trace element, geostatistical analysis showed three water groups: water with very low trace element concentrations encountered in boreholes and unpolluted surface waters, and waters with relatively high trace element concentrations such as Al observed in areas affected by gold mining activities and finally, polluted waters by Ni, Co, Mn and Cr observed at Sabodala. Results show that in eastern Senegal well waters are vulnerable and often affected by pollution.
A Shoulder Mass Revealing a Metastatic Prostate Cancer  [PDF]

Abstract: Introduction: A shoulder mass revealing metastatic prostate cancer is very rare. We report a case of metastatic prostate cancer diagnosed on a shoulder mass and treated with analgesic radiotherapy and chemotherapy and androgen deprivation therapy (ADT). Observations: A 66 years old patient was referred for a painful right shoulder mass whose biopsy and pathological examination found a Gleason 8 (4 + 4) moderately differentiated adenocarcinoma. The PSA level was 508.52 ng/ml. The patient was treated with analgesic radiotherapy on the right shoulder and chemo-hormonal therapy. At 2 years follow-up, the disease was well controlled. Conclusion: A shoulder mass revealing metastatic prostate cancer is not common. Local treatment of the symptomatic metastasis while continuing chemotherapy and ADT improves the quality of life.

Abstract: The title compound, [ZnCl2(C18H14N4O)], crystallizes with two molecules in the asymmetric unit, which differ in the tautomeric (neutral and zwitterionic) forms of the coordinating organic ligand. In both molecules, the ZnII atom adopts a distorted square–pyramidal geometry by two N and one O atoms of the Schiff base ligand and two Cl atoms acting as monodentate chloride anions. The crystal packing is stabilized by N—H...N and N—H...Cl hydrogen bonds, forming a two-dimensional network parallel to the ac plane.

Abstract: In the title complex, [MnCl2(C13H13N3)]·H2O, the MnII atom is in a distorted square-pyramidal environment, with an Addison τ parameter of 0.037. The coordination geometry is defined by three N-atom donors from the tridentate 2-(pyridin-2-yl)-N-(pyridin-2-ylmethylidene)ethanamine ligand and two terminal Cl atoms. Although the H atoms of the lattice water molecule were not located, O...O distances of 3.103 (7) and O...Cl distances of 3.240 (3) and 3.482 (4) suggest that hydrogen bonding is responsible for the stabilization of the crystal packing.

Abstract: We report in this paper the ground-state energy 2s2 1S and total energies of doubly excited states 2p2 1D, 3d2 1D, 4f2 1I of the Helium isoelectronic sequence from H- to Ca18+. Calculations are performed using the Modified Atomic Orbital Theory (MAOT) in the framework of a variational procedure. The purpose of this study required a mathematical development of the Hamiltonian applied to Slater-type wave function [1] combining with Hylleraas-type wave function [2]. The study leads to analytical expressions which are carried out under special MAXIMA computational program. This first proposed MAOT variational procedure, leads to accurate results in good agreement as well as with available other theoretical results than experimental data. In the present work, a new correlated wave function is presented to express analytically the total energies for the 2s2 1S ground state and each doubly 2p2 1D, 3d2 1D, 4f2 1I excited states in the He-like systems.The present accurate data may be a useful guideline for future experimental and theoretical studies in the (nl2) systems.

Abstract: Context: Technological advances have improved the toxicities of radiotherapy. We are evaluating the 3D technique in prostate cancer. Materials and Methods: Retrospective study from January 2015 to December 2015 with 29 files. Survival was calculated by Kaplan-Meier method. Results: We collected 29 patient records over the study period. The median age was 75 years with the following extremes: 54 years and 83 years. The median PSA level was 12 ng/ml with a range of 3.05 to 79 ng/ml. Gleason score analysis showed 6 patients (20.69%) with a score of 6 (3 + 3), 23 patients (79.31%) with a score of 7 including 12 patients (41.38%) with grade 3 and 11 patients (37.93%) with grade 4. The median dose delivered was 74 Gy, with a mean dose of 73.79 Gy and extremes of 70 Gy for the minimum and 76 Gy for the maximum. Hormone therapy was combined with radiotherapy in 17 patients (58.62%). Seven patients (24.14%) had grade 1 acute bladder toxicity and one patient (3.45%) had grade 2 acute toxicity. Late bladder toxicity was grade 1 in 5 patients (17.24%), grade 2 in 3 patients (10.34%) and grade 3 in 1 patient (3.45%). Late rectal toxicity, grade 2 in 3 patients (10.34%), grade 3 in 1 patient, was noted. Overall survival at 2 years was 100% and 89.65% at 5 years. Relapse-free survival at 2 years was 82.76% and 62.07% at 5 years. There were 3 deaths (10.34%) of which only one was related to prostate cancer. Conclusion: Radiotherapy, like surgery, is a fundamental option for the treatment of prostate cancers, particularly those that are locally advanced. It is gaining in importance with the
